Some heart medications (such as Nitrate) are taken in the morning and in the afternoon, and can be adjusted during fasting and iftar. Some medications need to be taken three times a day (such as Diltiazem). They can be taken once or twice a day in slow release form. 

5. Heart patients who have diabetes need to take special care. Not taking food and drink for a long time of the day can cause the blood sugar level to drop drastically. If you suddenly lose consciousness or feel dizzy, sweating profusely, you may suspect that your blood sugar levels may drop.

And the inhalers (such as Bexitrol Inhaler) that have to be taken twice a day should be taken during Sahri and Iftar. So heart patients be aware.
